The Job:
We are currently recruiting for an entry-level Community Support Specialist to join a dynamic team that serves underprivileged communities. The Care Coordinator will be responsible for providing direct support and coordinating care services for patients. The ideal candidate will have experience in direct patient care, or related education and possess strong organizational and communication skills with an empathetic mindset.
What You Will Be Doing As A Community Support Specialist:
Coordinate patient care activities, including scheduling appointments, arranging referrals, and ensuring follow-up care
Maintain accurate and up-to-date patient records in electronic medical record systems
Communicate with patients, healthcare providers, and insurance companies to ensure seamless care coordination
Provide individuals with physical, emotional, and behavioral support allowing them to actively participate in activities offered while encouraging personal choices and assisting in maintaining their personal safety
Provide transportation, in personal and company-owned vehicles, throughout the day to and from the individuals' homes and activity locations in the community
Educate patients on available resources and support services

Salary:
$40,000-50,000/yr + Benefits
Requirements To Be A Community Support Specialist:
Associate's Degree required; Bachelor's degree preferred in a field related to human services
Previous experience in a patient support role is highly desirable
Familiarity with medical terminology
